All persons making application to join our Practice list must do so by requesting an application form from our reception staff.
Please check with reception staff that you live within the practice boundary or click here to view a map.
Completed forms should be returned to reception to facilitate registration.
Should registration not be acceptable the practice will provide a reason, in writing, within 10 working days.
Patients may require a new patient check; the reception staff will arrange an appointment for this.
Patients may make an appointment with their doctor prior to a new patient check appointment if they wish.
It is important that we have up to date details in your medical file.
Please inform the surgery of any changes i.e. you change address or change name.
It can take 6 to 12 weeks for your medical records to arrive from your previous doctor and be summarised so it is really helpful if you are able to provide us with a “Repeat Card” from your previous surgery or your labelled medication boxes so we can be certain of the medication you are on. We may need to ask your previous GP for further proof of the medication regimen if it is unusual or complex so please make sure you request your first prescriptions from us in plenty of time (at least 5 working days).
Regular medications have to be “linked” to the medical condition for which they are taken. This is a Care Quality Commission (CQC) requirement. We are unable to link your medications until your notes have arrived and been summarised. Your medications will not appear on your “Repeat Card” until then. Please bear with us during this time. You are still able to order your regular medications with either a written request or by typing them into the Free Text box if using the online system.

Please do not stockpile or overorder medications – there is no need. We are only closed on weekends and Bank Holidays!
Please do not come in to collect medication or check if it is ready before 2 working days have elapsed – it wastes a lot of time for the dispensary staff!
Unfortunately, due to limitations with IT and our dispensing status, we do not issue any prescriptions through the Electronic Prescription Service (EPS).
Please provide stamped, self-addressed envelopes if you wish us to post your paper prescriptions back to you. Please note the 2 working day turnaround does not include posting the prescription so this will take longer.
Once a medication has left the Surgery, we are unable to reuse it but unwanted medication can be returned to the Dispensary for safe disposal.
Please do NOT bring “sharps” bins or boxes to the Surgery. You must contact the local council to arrange collection of these yourselves.
We are unable to sell over the counter drugs from the Dispensary.
We are unable to accept Hospital Prescriptions issued in outpatient clinics – these are usually a green A4 page with the Lloyd’s Pharmacy logo at the top and should be taken to the Lloyds Pharmacy next to the Outpatient Department at Derriford.
We are unable to accept Private Prescriptions or handwritten prescriptions from doctors who do not work at Wembury Surgery.

A Patient Guide to GP Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
